U.S. amazon is currently listing the title as “unavailable”. I assume this is in error. I’m ready to buy as soon as it’s available again!

Yes, this is an unfortunate and ongoing problem. I’ve had several emails from Amazon telling me it’s going to be fixed soon, but it’s been almost two months now–anyway, if you’re keen to read the book you can download it from this page:
